Sacramental Record Copy Request

- If you celebrated a Sacrament at St. John the Baptist Catholic Church, our Parish office staff may be able to assist you in obtaining a copy of your Sacramental record(s). Our office staff will need the following information to complete your request:
  
  First and Last Name
  
  Date of Birth
  
  Which Sacrament(s) you celebrated here at St. John the Baptist
  
  The date or date range of when you celebrated each Sacrament
  
  Sacramental record requests are free of charge. Please complete your request by completeing the record request form below:

  All requests for Sacramental records need to be made using the electronic form provided below. Email and telephone requests will not be accepted. Sacramental records must be requested by and may only be released to the following:
  
  Individuals who received the Sacraments
  
  Church or Religious Institution
  
  Parents of minor children (age 17 and younger)

  How long does it take to process a record request?
  Processing time may vary depending on your request. Generally it takes 1-2 business days to complete a Sacramental record search.
  
  To ensure a timely response, please be sure to provide as much information as you can recall in your records request.
  I'm getting married and my Pastor has asked me for a "Baptismal certificate with notations", what does this mean?
  If you are getting married in the Catholic Church, you will need to submit copies of your Sacramental certificates to the Church in which you will be celebrating your marriage.
  
  The easiest way to obtain records of your Baptism, First Communion and Confirmation is by receiving a copy of your Baptismal certificate with notations.
  
  Here at St. John the Baptist we always complete a Baptismal certificate request with notations of any other Sacraments you've celebrated that are on record here at our Church.
  I am unable to visit your Church to pick up my certificate, how can I receive a copy of my certificate of Baptism, Confirmation, etc.?
  Upon completing your request, our Parish Office staff will email an electronic copy of your certificate to the email address you've provided.
  
  We will also mail the original copy of your certificate to the mailing address listed on your request form.

How long does it take to process a record request?

I'm getting married and my Pastor has asked me for a "Baptismal certificate with notations", what does this mean?

I am unable to visit your Church to pick up my certificate, how can I receive a copy of my certificate of Baptism, Confirmation, etc.?
